Subject: unable to add catalog item

I am trying to add new catalog item but after enterin the credentials and push apply the page goes white and nothings comes later. i check the catalog and see that it was not added

Please open a service request on this issue and attach a zipped copy of the \SysAidServer\root\WEB-INF\logs folder after re-creating this issue.
