I have been at this for hours!!!

I have been following the direction in this article http://www.ilient.com/Sysforums/posts/list/251.page)and cannot get this to work. I got it to show up once but it was way out of line and goofed up some other things. I attached a copy of my submit user form html file if someone could add this and send it back I would be greatly appreciative . here is the html from the article above:

if possible I just wanted it to be added under the main asset and before the send screen capture option

I had to use a lot of breaks in mine for alignment. I'll get you a sample of mine..

thanks for the help but I think my problem is I am not placing in the correct spot in the html file, can you help with where to place it?

has to go below the category/subcategory/thirdlevel code. You have to make a template folder and copy the submit service request form to the template folder and edit that one. Then Customize>Appearance>Tell the UI where the folder is and hit save...
